Individuals can request transcripts 3 ways:

1. Written request by U.S. Mail
2. Written request by FAX; our fax number is 262.650.4851
3. Complete transcript request form in Registrar’s Office

We CANNOT take transcript requests over the phone or by email and we CANNOT fax transcripts.

Transcript requests should be sent to:

Carroll University
100 N. East Ave.
Waukesha, WI 53186

Information needed in transcript request:

1. Student’s full name, address, and daytime phone number
2. Dates of attendance at Carroll College
3. Student ID# or Social Security Number
4. Complete name and address where the transcript is to be sent.
5. Type of transcript – Please indicate official or unofficial with each request.
6. Signature, this is very important. We CANNOT process your request without a signature.

Cost:
The cost for each OFFICIAL transcript is $4.00. You may pay this by mailing a check with your request or in-person when you PLACE the order in the office. You may also pay via credit card. If paying by credit card because you are faxing the transcript, you may call the business office at 262-524-7337 with your card number and expiration date. We cannot process transcript orders unless payment is received at the time the order is placed.

Processing Time:
Transcript requests generally take one to two business days. Transcripts are sent via U.S. Mail. We cannot process transcript requests for students or alumni with student accounts holds. Failure to include all the necessary information will delay processing time.
